Two of the new codes 2010 for appendicitis, K353 (Akut appendicit med lokaliserad peritonit) and K358 (Annan och icke specificerad akut appendicit) have by accident got the property 06C03 (Complicated appendicitis) which leads to DRG 166N (Appendektomi, komplicerat) if an appendectomy is performed.

That K358 has got the property 06C03 is obviously wrong. That K353 has got the property 06C03 is also wrong since some degree of peritonitis (at least in the visceral peritoneum) always can be considered to exist in cases that are operated for appendicitis.

Thus it can be correct to use the code K353 in almost all cases that are operated for appendicitis and there is an obvious risk for that almost all appendectomies will be grouped to DRG 166N and very few will be grouped to DRG 167 (Appendektomi, ej komplicerat).


We suggest that the property 06C03 is withdrawn from the diagnosis codes K353 and K358. We consider this to be an error that can be corrected by the Centre without prior discussion in the Nordic Expert network but the case has to be published on the NordDRG Forum for information and documentation.

This is basically an error of the Swedish ICD coding. If K35.3 and K35.8 are supposed to indicate unspecified appendicitis, they should be sub codes to K36 which is the uncompleted appendicitis in WHO classification. However, we will now map these codes (K35.3 and K35.8) to K36 and get the results that are wanted.
