A patient with P071A (Annan låg födelsevikt, 1000–1499 g) is grouped to DRG 387N (Nyfödd, födelsevikt 1000 -1499 g,) with the weight 9,5813. If P220 (Respiratory distress syndrome hos nyfödd) is added the case is grouped to DRG 388C (Respiratory distress syndrome hos prematur nyfödd) with the weight 8,7235. (Swedish weights 2010.) Respiratory distress syndrome is a “heavy” secondary diagnosis and it should not cause a DRG with lesser weight.
It seems to be a hierarchy problem. According to Swedish cost data the hierarchy in Drglogic should be changed so that DRG 388C comes after DRG 387N. The number of observations in DRG 388C is very small, however, so therefore Finnish cost data has to be studied as well.
