The Swedish National Board of Health and Welfare (SOS) makes own definition tables parallel to the process at the Nordic Casemix Centre (NCC) and the current problem was discovered during routine comparison between SWE2014CC-SOS-PR02 (SOS version) and SWE2014PR0F (NCC version) in the autumn 2013.

When we compared the NCC and SOS versions for 2014 we remarked that 38 code pairs were missing in the table Compl.excl. in the NCC version. Martti answered: “Technically this is not needed. If a codepair is registered as principal diagnosis, exclusions are taken for dagger and asterisk code.”

If we understand Marttis comment correctly it means that code pairs (asterisk + dagger) are separated and divided into two separate codes and this is hard coded in the grouper? We think that this principle is incorrect. If we have decided that a code pair shall have other properties than the asterisk code, the code pair must be handled as one unit in the entire grouping process, which means also when the grouper searches in the table Compl.excl.

The 38 code pairs in the table Compl.excl. that excisted in NordDRG 2013 but deleted in the NCC version for 2014 should be reinserted. The code pairs are specified in Suggestion CPK ID 525.xls.

In the case of code pairs when they are used, NordDRG collect the properties from both the manifestation code and etiological code. The diagnosis category (and as part of it the MDC) is taken from the manifestation code. The complication category is also taken from both codes. Thus if one of the codes belongs to a comlication category, the pair will automatically belong to it.
Code pair needs to be defined separately only if the property we wish the pair to have is not the diagnosis category of the manifestation code and if any other property is not present in either of the codes.

This beeing said, quite many of the listed propeties seem to be present in the current defintion tables.

Comment Expert Group 2014-03-24-25

Martti proposed to clean complication exclusion list from properties that are not the same in all countries.
Proposal to take the codes out is accepted by all.

Sweden proposes to make a new case about including the codes for 2016.
