Problem

The Swedish National Board of Health and Welfare (SOS) makes own definition tables parallel to the process at the Nordic Casemix Centre (NCC) and the current problem was discovered during routine comparison between SWE2014CC-SOS-PR02 (SOS version) and SWE2014PR0F (NCC version) in the autumn 2013.

In the NCC version 2014, the code Z412 (Routine and ritual circumcision) has lost its OR property that it used to have. OR=1 is necessary, however, for the cases to be grouped to DRG N20/343 (Circumcision) according to the present Drglogic.

Suggestion

OR=1 is reinserted for the diagnosis code Z412 as specified in Suggestion CPK ID 546.xls.

National organizations may decide to introduce the change in their own version.
Otherwise the organization is NCC.

DRG change

Cases with Z412 that wrongly have been grouped to a conservative DRG during 2014 will correctly be grouped to DRG N20/343.

Technical change

Technical description in text format.
There must also be technical changes according to the NCC Excel template. This Excel file should be named “Decision” plus the ID of the case.

This is the last case of a diagnosis with OR=1 property and it would be logical not to have such codes.

There are 2 problems in the coding system that cause this situation:
1) The dx has procedure information that does not belong to the diagnosis classification.
2) The intervention code that should be used has diagnostic information that makes it’s use in this situation problematic
KGSH10 ‘Operation for phimosis’

KGSH10 is obviously called ‘circumcision’ but probably for political reasons it has been formulated this way.
I agree with Mats, we cannot change the basic coding systems and seems that we have to maintain the current model.

Comment Expert Group 2014-03-25

Change will be to keep the rule, keep the property as done in year 2013 grouper until the primary coding is corrected.
Change the code text to "circumcision" (Sweden).
This proposal is accepted.

To complete the analysis the code KGSV20 ‘Ritual circumcision’ is present in all other versions than Swedish. Since this is still not a problem in any other country, the properties of Z4120 will be retained in all versions.

Technical change

Z4120 will retain properties OR=1 and PROCPROP 12S05

DRG change

Cases with ritual circumcision are assigned to correct DRG even in the absence of proper intervention code.

The procedure property was 14S05 that is obviously wrong. The correct property is 12S05 'Circucision'.
I have corrected the decission above accordingly.
