Problem
Old case #168 on Forum from Norway 2010.
The Expert spring meeting 2010 accepted the Norwegian proposal that Cochlea implant surgery is grouped to one single DRG independent of length of stay. It was then also stated that “This change will not affect Sweden.”
We have now noticed that it does affect Sweden. Outpatients with Cochlea implant surgery are grouped to the inpatient DRG C02N/049B Insertion of cochlear implants.

The last character in the Swedish DRG codes is specific for the type of care and we prefer to be able to see the difference between daycare and inpatient care in this easy way throughout the system.

Analysis

This is a matter of principles and not an economical issue. Sweden wants that outpatient DRG and inpatient DRG are separated.

Suggestion
The Swedish National Board of Health and Welfare – 2014-12-18
Current 049B/C02N is devided to 049B/C02N for inpatients and 049O/C02O for outpatients

To be introduced in NordDRG SWE 2016.

DRG change
A few outpatient cases will be grouped to DRG C02O instead of C02N

Technical change
The present rule for DRG 049B/C02N is among the outpatient rules must be substituted by an usual outpatient rule for DRG 049O/C02O with Discharge set to 'N' and Duration to '<1'(for SWE version).
An inpatient rule for DRG 049B/C02N is added before the current rule 403D0020 (DRG 049A/C05N). This is a copy of the present rule for DRG C02N.

The basic proposal is edited.
The statement 'This change will not affect Sweden' is not understandable from current situation. It is obvious that it must affect even Sweden.
It is difficult to see from current tables where these cases were assigned before 2011 and Case #168 the outpatient cochlear implants. However there are obviously no good options.
In principal the simple economic analysis stating how many cases of this type there will be and what is their mean cost per patient, is necessary. Even in this case we should follow our rules for creating new DRG's.
The expert group needs to decide, whether this is interesting for all versions!
