Description

Problem
Swedish NordDRG users have noticed that the procedure code DF012 ‘Toraxkompression’ (NCSP+ code FXXA00 ‘Closed cardiac compression’) affects the grouping, but DF028 ‘Hjärtlungräddning’ (NCSP+ code WAA900 ‘Resuscitation’) does not. This is very inconsistent since cardiac compression is a part of resuscitation.
Analysis
NPK, Sweden, 2016-02-17
It is obvious that DF012 and DF028 should have similar grouping properties and then PROCPR 40V01 and PROCPR 15S02 should be added to DF028.
Looking at these codes we found that DF017 ‘Mekanisk toraxkompression (ej manuell)’ and DF030 ‘EECP-behandling’ (both with NCSP+ code FXXA05 ‘Mechanical heart compression’) have CC = 1, so we wanted to analyze if DF012 and DF028 also should have CC = 1. The analysis is presented in Appendix C436.xlsx. We found no evidence to give CC = 1 to DF012 and DF028. As a matter of fact we found no evidence for DF017 to have CC either, but the number of cases was too small to draw any firm conclusions.
The method used in this analysis is new and it could be interesting to hear our Nordic colleague’s opinion about it.
Suggestion
NPK, Sweden, 2016-02-17
The procedure properties 40V01 and 15S02 are added to the procedure code DF028 ‘Hjärtlungräddning’ (NCSP+ code WAA900 ‘Resuscitation’). The property 40V01 is used only in the Swedish outpatient grouping logic and the property 15S02 is valid only for neonatal care.
Technical changes are specified in the Excel file Suggestion C436.xlsx.
DRG change
Swedish outpatient cases with the procedure code DF028 that used to be grouped to DRGs for visits without any significant procedure will instead be grouped to DRG X01O ‘Intensivvårdsåtgärder, besök’, like cases with the procedure code DF012. Neonatal cases with the procedure code DF028 will also be grouped like cases with DF012.

I support this change.
For 15S02 it affects all versions, 40V01 is a Swedish property.

This case was discussed at the Expert Network meeting 2016-03-14 – 15 and it was decided to be implemented and therefor I attach the file Decision C436.xlsx (which actually is just a copy of Suggestion C436.xlsx).

The proposal was accepted for all versions. The affected national codes are listed in the NCSP+ sheet of technical changes.
