If a percutaneous occlusion of a. Lienalis is done because of a spleen injury, code S360 the DRG will be 477. We suggest to change this.

We suggest to add procpro 16S01 to procedurecode PCY22B. This will give either DRG 392 Op på milten pga sykdom i HDG 16 > 17 år, or DRG 393 Op på milten pga sykdom i HDG 16 0-17 år depending on the age of the patient.

Finnish National DRG-centre 2017-3-3

To Norwegian code PCY22B corresponds PCST20 in NCSP+ and PCT16 in Finnish version (Injection of therapeutic agent into or percutaneous occlusion of coeliac trunk and branches).
Two cases with code PCT16 in Finnish data, main dx I714 and I728, grouped to DRG 479O Other vascular procedures, short therapy due to the PROCPROP 05S15 Other vascular procedures.

The change would be supported. It would have minimal impact in Finland though (based on current data).

Sweden has the same problem, but we do not have DRG 392 'Operation of spleen..’. Addition of procpro 16S01 would, in Sweden, lead to DRG R01N ‘Extirpation of spleen’ which would be wrong medical description. Addition of procpro 16S02 would probably be better, this would lead to DRG R05N/394X Other o. r. procedures of the blood and blood forming organ, but we do not want to make a change now. First we need more thorough analysis.

Expert group accepted the change for NordDRG Nor only.
To acchieve this a new NCSP+ code for 'Occlusion of spleen artery' PCST24 is added to NSCP+.
This gets all properties ov PCST20 + a new property 16S10 'Percutaneous operation on spleen'
In addition 3 new lines for the exixting DRG's are needed. These are only valid for NordDRG Nor.
If Finland wants to join this model, it can be arranged.

We would keep it the same in Finnish version. Thus, no changes in 2018.
We will check the case again and make more thorough analysis when the data of 2016 will be available.
