Separating short therapy diabetes cases from DRG 910O

Currently the short therapy diabetes cases are grouped in Finnish version into DRG 910O Endocrine, nutritional and metabolic disease or disorder, short therapy w/o significant procedure. Given the burden diabetes gives to health care systems and bearing in mind the importance of follow-up of diabetes patients, Finland would like to have clinically meaningful group for outpatient diabetes patients and separate the cases from other cases in DRG 910O.
This change follows the changes which have been made already earlier in Swedish and Norwegian version.
For analysis, the data of five Finnish university was used from year 2018.
The analysis shows that there is no significant difference in mean cost of 259O and 910O cases (14% without trimming, and 6% after 1% trimming). However, the clinical meaningfulness plays more important role in that change.
In both groups the V% improves significantly after 1% trimming (in DRG 295O 111%->66% and in DRG 910 194%->79%)
Based on the analysis, we suggest introducing DRG 295O for short therapy diabetes cases also into Finnish version.
After the suggested changes the short therapy diabetes cases previously grouped into DRG 910O will be grouped into DRG 295O Diabetes, short therapy. In definition tables, new rule (124D2010002) will be added above the current rule (110D109960) of DRG 910O.
Appendix 1. Cost analysis is based on five university hospitals’ data of year 2018, grouped with FIN 2020PR1 grouper.

The proposed change was accepted for Finland (and for Norway). The changes will to extent possible use the same logic rules as existing Swedish model The DRG coding will obviously be different and that is taken care int the national DRGnames table.
The technical changes will be reported based on new NDMS.
