This case presented here is about the Swedish updates for ICD-10 codes, procedure codes.

ICD-10 - Has new codes, removal of codes, updates on texts.
Procedure codes (not including vascular codes) – Has new codes, removal of codes, updates on texts.
ICD-10 and Procedure codes (not including vascular codes) are presented in one Excel-file 'TC_SWE updates ICD, Procedure codes 2022'.

Procedure codes concerning vascular codes - The changes that has to be done for vascular codes are presented in one separate excell-file ‘Vascular proc updates for Swe 2022.xlsx’.

We are sorry for this dubble Excell-file presentation.
The excell-file ‘Vascular proc updates for Swe 2022.xlsx’ includes all changes for vascular codes. They are more difficult to handle. So we decided it was better to present by themselves.
It is easy to understand when reading the sheet ‘read me’. There it are a good description of the different sheets and I think all information needed is included there.

These are all the codes that we have gotten until 2021-06-18.

For the last couple of years we have gotten more codes later than 1 June, if that happens we will put them on Forum as soon as we can.

Unfortunately, there are two errors in the file “Vascular proc updates for Swe 2022.xlsx”. The codes WAI11 and WAI33 that we had received from the primary classification team were wrong. The correct codes are VAI11 and VAI33. These errors have been corrected in the new file “Vascular proc updates for Swe 2022 - rev 2021-06-22.xlsx”.

A comment concerning the features of swe code Z488A Annan specificerad kirurgisk eftervård vid sjukdom i cirkulationsorganen.
According your suggestion code Z488A should be given following features:
DGCAT 05M20 Aftercare for cardiovascular diseases
DGPROP 05X99 Diseases and disorders of the circulatory system
MDC 05 Diseases and disorders of the circulatory system
To be able to add DGCAT 05M20 to Z488A, it has to be created and added to the DGCAT list in NDMS. 05M20 does not exist at the moment.
Given that dg codes Z488A and Z488Z Annan specificerad kirurgisk eftervård vid sjukdom med ospecificerad lokalisation have different features, a new ncsp+ code will be created for Z488A.

In addition to previous comment (on Sept 1), new DGCAT has no impact on grouping if not added to logic table. For sake of transparency it would make sense to create a new case with TCs related to new DGCAT and grouping rule.

In the sheet 'ncsp' I have found some things we have quesstions about. Please read the enclosed Excelfile, sheet ncsp and comment if I do not understand it correctly.

Thank you for feedback concerning the changes in procedure classification and respective mapping and properties. Based on this, in some cases the mapping was corrected or properties added, in other cases the clarification was added. See more in attached file. If no additional comments, the updates of new procedure codes should be ok.
We will provide the feedback concerning vascular surgery codes asap.

Please find attached the feedback to updates of vascular surgery codes. Sorry for being late with this.
At this point no new ncps+ codes were created because it would not have changed the properties compared to current mapping. If there were new or different properties, the mapping could be changed.
Please have a look and if any additional comments or need for clarifications, please leave them in this ticket.

You don’t have to be sorry. There was a huge number of codes to consider.
As far as I can see, your mappings will result in desired grouping properties and we are happy with that.

However, it is a pity that you are so restrictive in creating new plus codes. If the only purpose with NCSP+ is to allocate grouping properties, the system has a low clinical relevance and it can probably not be very useful for the future mapping to ICD-11.

Concerning the codes for “injection of therapeutic or sclerosing agent”, it is true that sclerosis is also therapy, but in this context there is the same difference as for thrombolysis and occlusion. The technical procedure, the injection itself, is almost the same but the indications are very different and the costs for patients with thrombolysis and patients with occlusion are probably quite different. Unfortunately, this is not possible to find out from the cost database as long as we have the same code for injection of therapeutic agent (thrombolysis) and injection of sclerosing agent (occlusion). Norway has realized that earlier and introduced separate codes and now Sweden will follow them.
However, the question how differentiated NCSP+ shall be is another subject and perhaps we will make a new ticket on the Forum for that.

Just a minor correction. The day before yesterday, our team for primary classifications announced changes in the texts for two of the new vascular surgery codes. The codes and updated texts are:
VMJ42 Perfusion av isolerad arm från djup ven (vena subclavia/axillaris) till armartär (a. subclavia/axillaris)
VSJ13 Perfusion av isolerat ben från vena femoralis till a. femoralis
I am sorry for this late change but it is beyond the control of the DRG team.

The reason for being restrictive in creatin new NCSP+ codes is very practical:
1) properties of new NCSP+ codes need to be defined wheras natioanl codes linked to existing NCSP+ codes get thei properties automatically from existing codes
2) The number of new Swedish codes is substantial. It is possible that also other codes than thsose proposed, shouls have special properties.
Since the proposed new codes would have properties of existing codes, it is propaply better to to analyze the need for properties before we extend the NCSP+.
The special question aboudt "sclerosis", "occlusion" and "thrombolysis" needs needs common discussion in the expert group. It is a typical example of the problem how detailed NCSP+ needs to be.
If the NCSP+ will be substituted with ICHI based model, the possibility of adding details to coding by using combinination fo codes as a "cloud" may also be concidered.
